Home Nursing

Skilled nursing services are provided in the person’s home. Services may include care related to a specific condition or coordination of medical services. It may also include accessing other medical services. The nurse follows a prescribed plan of care that is based on the person’s medical condition. Education about medical needs and supports may be provided by either primary care provider or the DDD District Nurse.
